Position / Title:
Certified Application Counselor STATEMENT OF PURPOSE Southwest La Area Health Education Center is currently looking to fill Certified Application Counselor positions in specific regionally designated areas across the state. Our goal is to decrease the amount of community members who may go without insurance through strategic outreach methods.
The Certified Application Counselor will devote 100% of their time to this project. The Certified Application Counselor will be responsible for processing and collecting data for our research partners as well as the Southwest Louisiana Area Health Education Center data and submitting required information to the program director as requested.
MAJOR TASKS, DUTIES, AND RESPONSIBILITIES - Conduct outreach regarding potential loss of Medicaid coverage, and processing of Louisiana Medicaid applications and renewals.
- Process lists of Medicaid recipients on a monthly basis to make contact regarding renewal needs.
- Take applications, assist applicants in completing the application, provide information and referrals, obtain required documentation to complete processing of the application, assuring that the information contained on the application form is complete, and conduct any necessary interviews and follow-up.
- Track and report activities performed on set schedule in accordance with contract terms and requirements.
- Provide Medicaid outreach and application assistance according to predetermined schedule of activities.
- Provide referrals to Louisiana Department of Insurance, health insurance ombudsman or any other appropriate State agency, for any enrollee with a grievance, complaint, or question regarding their health plan, coverage, or a determination under such plan or coverage.
- Provide referrals to Navigators for a Health Louisiana for Louisiana Medicaid Ineligible participants to assist consumers in obtaining health insurance coverage.
- Utilize brochures, flyers, and other marketing materials with information about the project.
- Utilize flyer templates where specific community information can be dropped-in to be unique for each outreach activity across the state.
- Participate in all staff training, licensing, meetings, etc.
- Work with Program Director and other staff on the Community Mapping for Outreach efforts and data processing/reporting.
- Other reporting duties as identified on an ongoing basis by the Program Director.

- Cannot have conflicts of interest, financial or otherwise.
- Demonstrated experience building community relationships.
- Knowledge of computer software applications.
- Well-developed oral and written communication skills.
- High School Diploma
- Experience working with data
Physical Demands and Working Conditions - Physical mobility
- Reliable transportation, current license, and auto insurance.
- Some local travel to and from pre-designated sites.
- Non-traditional work hours that could include weekends.
- Remote position – Weekly schedule of activities planned required.
- Schedule is subject to applicant’s daytime availability and assignments.
- This is a Full-Time Position – 12 Month Contract terms.
The Certified Application Counselor – will be responsible for participating in all training activities required by the contract, SWLAHEC, Louisiana Medicaid, and the State of Louisiana as well as any additional training needed to perform CAC duties. This includes but not limited to:
- Louisiana Medicaid Application Center – Trusted User Certification (Must be certified to conduct CAC job duties)
- Navigators for a Healthy Louisiana direct referral process Training
- Advocacy Center Training / Working with Disabled Communities
- Cultural Competency Training / Working with Non-English-Speaking Communities
- Community Targeted Outreach Training
- Any state certification training or licensing requirement is needed to operate as a Certified Application Counselor in the State of Louisiana.
- Must comply with Federal and State requirements and the terms of funding contract.
- Adhere to all SWLAHEC policies and procedures.
