This individual will assess patients' knowledge of diabetes and develop tailored educational plans that include lifestyle modifications, medication management, and blood glucose monitoring. The Diabetes Care Education Specialist will also work closely with healthcare providers to coordinate care and ensure that patients receive comprehensive treatment.
