Job Details

• ALL APPLICANTS MUST HAVE 1 year Certified Home Care experience.
• 1 year Case Management Experience.
• Ability to travel to cases as assigned.
• Weekend rotations are once every 6 weeks.
• As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
• Patient care is delivered within organizational philosophy, policy and standards of community health practice.
• Required to perform admission processes and case management responsibilities in accordance with MGB Home Care’s policies and standards of practice.

Schedule Information

• Weekend rotations are once every 6 weeks.

Additional Details

• Submit a color photo taken in past 6 mos. No selfies, no filters. Plain Background. Symmetrical headshot, shoulders up, look straight/no tilted head. No eyeglasses.
• MVR check to be run by background check company within 45 days of start. Required for all home care positions.
• Primary Source Verification of professional license and/or certification, as required for the position. Within 30 days of start date and updated upon expiration.
• N95 - within 365 days to start date and updated annually.
• Within 30 days of start. Required by MA DPH for all employees.
• A completed primary COVID-19 vaccination series that is approved by the FDA or the World Health Organization (WHO) OR an up-to-date COVID vaccine if a primary series was never received or completed is required.
• Documentation of two measles vaccines, two mumps vaccine, and one rubella (German measles) vaccines or two MMR vaccines.
• Strongly Preferred. Record of 3 Hep B vaccines OR Proof of immunity via blood test.
• Vaccine required during flu season Sept 15th - April 30th.
• Must have documentation of TB skin test screening within 3 months of start date and updated if >6 month break.
