Job Description

Job Summary

Provides support to emerging adults between the ages of 18-24 who are recovering from mental illness and/or substance use to enable them to achieve their aspirations for independent living. Actively encourages residents to be involved in every aspect of household management.

WORKING CONDITIONS

  • The position involves working with residents who have behavioral health concerns and who may at times exhibit behavioral issues. 
  • Potential exposure to communicable diseases including the common cold, flu, and hepatitis.
  • Will have a desk and workstation 

PHYSICAL DEMANDS

Must be able to sit, walk, stand, twist, and bend for brief periods of time. Occasional reaching and or grasping using hand and/or arms.  May be required to move or lift up to 20 pounds. May be required to ascend/descend stairs. Regular use of hands requires dexterity in using the telephone, computer keyboard, or mouse. Ability to handle disruptions due to requests from members.

Job Responsibilities

ESSENTIAL FUNCTIONS

Interact one-on-one and in group settings with members.

  • Form relationships with members by talking with them and reading their history and assessments in the charts. Socialize with them on each shift to build rapport and a trusting, friendly bond.
  • Know the goals of each member by reading the Recovery Plans. Become familiar with each goal and the rationale for it. Address the goals with the member during the shift. Inform the Emerging Adults Housing Supervisor of additional goals as necessary.
  • Create learning opportunities and teachable moments with members to promote further knowledge and independence.
  • Supervise members individually as they take medications. Discuss the medications, the proper dosage, and the time it is to be taken. Educates members on how to take their medication independently.
  • Assist members with daily skills such as cooking, general housekeeping, hygiene, and other activities of independent living. 
  • Encourage members to participate in community activities offered by Park Center and have the members sign up on the sign-up sheet.
  • Ensure the safety of all members by checking regularly during shifts. Know where the members are and what they are doing throughout the shift.
  • Ensure that member rights are enforced.

Public/Staff Relations.

  • Utilizes voice and email
  • Communicate regularly with other staff to meet the needs of participants
  • Develops appropriate working relationships with other mental health, social services, and    rehabilitation agencies 
  • Works as a team player with other Park Center staff
  • Ensures confidentiality of all participants by not disclosing information about participants to anyone outside of Park Center staff.

Maintain daily operation of living facilities.

  • Ensure that the house is kept neat and clean by prompting and/or assisting members in completing assigned chores daily. 
  • Maintain members’ personal food in designated areas, keep clean, organized, and in containers.
  • Notify the Emerging Adults Housing Supervisor of maintenance issues as they arise.
  • Creates weekly grocery list with input from members.
  • Performs and documents monthly fire drills.

Recordkeeping Duties

  • Document daily progress notes on each member in EHR 
  • Document when medication is given by initialing the medication log.
  • Complete night security checks of the house when applicable.
  • Clock in and out using the electronic system.
  • Complete annual evaluation.

  • Collaborate with staff, members, and the community to gain the perspectives of others having diverse opinions, abilities, values, beliefs, perspectives, and ethnic or cultural backgrounds. 
  • Take deliberate steps to increase your own cultural competency by attending trainings, events, discussions, workshops, etc.

Other duties as a member of Park Center Housing Staff:

  • Participate in quarterly supervisions with the Emerging Adults Housing Supervisor.
  • Complete Essential Learning courses by the deadline.
  • Keep up with current issues and correspondence by reading emails on each shift.
  • Assist with housing events and extra activities as applicable.
  • Maintain good attendance and punctuality. Follow the policy for vacation and sick time.
  • Follow all Personnel Policies and Procedures.
  • Follow HIPAA confidentiality guidelines.
  • Follow emergency procedures (i.e.: resident crisis, injury, or weather).
  • Follow the On-Call procedure.

Qualifications

SKILLS AND QUALIFICATIONS

  • Bachelor’s degree in social work, psychology, or social services-related field and one year of experience in a mental health or social service setting
  • High school diploma or equivalent, with five (5) years experience in a mental health or social service setting. One year of which was working with youth or emerging adults. 
  • Ability to work independently and with a team 
  • Ability to meet deadlines
  • Strong verbal and written communication skills
  • Computer proficient in Word and Excel 
  • Strong professional and ethical behaviors 
  • Must have a valid Drivers’ License
  • Must be able to show proof of personal automobile insurance 
  • Must be insurable under Park Center’s automobile insurance
